The participant is responsible for submitting the Physician Results Form as part of the wellness program. This form must be returned to Wellworks For You and can be found on the Wellworks Portal.
PLEASE NOTE: If you have a spouse enrolled in the Air Group medical/prescription drug plan, BOTH you and your covered spouse must complete the action items in order to earn the financial incentive.
REMEMBER: Preventive care is covered 100% when seeing an in-network provider.

New Hires and Newly Benefits-Eligible Employees:
To earn the wellness premium reduction effective June 1, 2027, employees and spouses, if applicable, must submit a completed Physician Results Form through the member portal in time to allow up to 10 business days for processing. All requirements must be submitted and confirmed by May 1, 2027, regardless of hire date or benefits eligibility date. Employees hired or becoming benefits-eligible after May 1, 2027 will remain at the non-wellness payroll rate and may earn the next available incentive effective June 1, 2028.
